I have been asked this question numerous times, not only by patients and their relatives, but also by some nurses. People would think that when we remove the stomach, the patient would not be able to eat normally. This is not correct.
Whenever we remove part or even the whole stomach, the surgeon restores the continuity of the digestive system by connecting the remaining part of the stomach to the bowel. And in case of the complete removal of the stomach, the jejunum is connected to the bowel.
After the initial recovery from surgery, which usually takes few days, patient would be able to eat normally. But sometimes the size of the meal is reduced because the capacity of the remaining stomach or the bowel in case of complete removal of the stomach are smaller than the capacity of the whole stomach.
We advise these patients to eat more frequent than the usual 3 meals a day. There would be no long term serious side effects and patients would enjoy a normal life. Vitamin and mineral supplementation, particularly vitamin B12, would be prescribed to the patients.
Although stomach and oesophageal resections are major surgery, but they do not cause much pain after.
Doctor Ahmed Hassan has made a questionnaire on 120 consecutive patients who had oesophagectomy or gastrectomy under his care. 98% of patients reported mild to moderate pain, which was easily controlled by simple analgesia. Nearly all patients did not need any painkillers more than paracetamol when they were discharged at home.
Doctor Hassan explained why pain after this major surgery is not a big issue. We have been using the keyhole laparoscopic techniques for more than 2 decades and this significantly reduce the tissue damage used to happen in case of open surgery.
We insert a fine line catheter at the end of surgery which constantly deliver local anaesthetic to the site of surgery and this relieves the pain. We use enhanced recovery protocols, which makes the post operative period go smooth and enhances the recovery.
Oesophageal and stomach resections are challenging surgery for a serious disease, which is cancer of the stomach or the oesophagus. If the operation is done properly and by an experienced surgeon, the outcomes are usually very good. The main obstacle to stomach and oesophageal resections for cancers are actually the stage of the disease.
Unfortunately, many stomach and oesophageal cancer patients present late when the disease has progressed or spread to other organs. Surgery is generally not recommended if the cancer has spread to other organs because there would be little merit in performing a major surgery without being able to cure the disease and remove different organs when the cancer has spread too. Patients with cancer spread are usually treated by chemotherapy plus or minus radiotherapy.
When scans and different staging investigations confirm that cancer has not spread outside the stomach and oesophagus, cancer resection by surgery offer the best chance of cure.
Professor Ahmed Hassan has treated more than 1200 patients with cancer stomach and oesophagus and has introduced numerous modifications and innovative techniques to the stomach and oesophageal cancer resection surgery. His data for consecutive patients in the last 10 years showed that 98% of patients survived this surgery. And less than 10% has developed complications after surgery, which were simple and easily corrected in the majority of cases. The vast majority of patients recovered well and were able to go home safe 4 to 5 days after surgery.
